Registered Nurse Case Manger/Delegating Nurse, Full Time

The company opened its doors in 1989 and has grown over the last 30 years.
We currently have a little over 90 employees, 35 residents who live in our community as well as two day programs.

We currently have an opening for a RN Case Manager to join a strong team of nurses.
Collectively the team of 2 LPNs & 3 RNs has more than 50 years of experience providing care at CCR! This is a unique nursing opportunity that offers flexible work hours and a positive work/life balance.
Every nurse on the team is both valued and supported.
Our RN Case Managers have small caseloads which allows them to be involved in every aspect of an individua's life.
This position offers a sign on bonus of 500 dollars after 6 months of employment and offer s a generous benefit package , including 9 paid holidays, 3 weeks of PTO, in addition to insurance and retirement benefits Essential job duties include:
45 day nursing assessments Care plan development Staff training and oversite Medical liaison Coordination of care Site visits Patient advocacy.
On call triage every 5 weeks Requirements essential to this job:
Unrestricted RN license in the state of MD or multi-state license Unrestricted driver's license Must live within 30 miles of North East, MD Must be able to read, write, speak and understand the English language Previous experience as a RN Case Manager/Delegating nurse (preferred) but not required Efficient in Microsoft Office and G mail Excellent attendance history Able to lift up to 60 lbs.
